Kosh v2.0 (with Breadcrumb)

Kosh is now at v2.0, it will stay like this for a while.
Features:
- Left, right or both navigation
- 100% wide with scalable content area
- Easy to change the colours via CSS
- Breadcrumb trail (now works with serendipity_event_staticpage)
