Contact Form

Am trying to add a contact form to my website, but am only getting a blank page.

Is there supposed to be a standard form with this plugin, or is the form something I have to design?

What template are you using, and what is the path to the contact form?

If you did not change the default, it would be at http://yoursite.com/s9y_install_folder_ ... tform.html
