Wow.
I've never seen a content change cause a 404. If that's the only thing in your test.php, I suppose Serendipity could be writing the 404 into the headers, but we normally redirect to the blog main page instead.
Maybe the redirection isn't working because it's a Windows server? Which server is this running on? Does it respect .htaccess, and if so, what's in the .htaccess? Which redirection method are you using in your configuration?
Sorry there are so many questions. It's just such a weird problem, and I want s9y to work in all environments.
