Yes, I set up /serendipity/uploads/images for my images. I can upload no problems using the media manager and it creates the thumbs also in that directory.
Sadly you didn't properly perform the upgrade. Did you follow the steps mentioined in the FAQ? You are missing a database upgrade.
Please redo the upgrade like outlined in the FAQ.

IT is a big issue! You are missing database columns that are required for porpoer operation.
Did you see the upgrader when you upgraded ?Did it tell you that it will execute some SQL statements? And did you tell the installer to go ahead with these? any error messages on the following screen?
Make sure your SQL user has privileges to execute "ALTER TABLE" statements.

The mediadatabase upgrade for the realname column happened much earlier then the verison you backgraded, it seems.
The realname column was introduced somewhere in serendipity 1.1, so you might need to browse through the sql/db_update*.sql files to check if those listed columns exist.
You might need to backdate your version to serendipity 1.0 (like the FAQ mentions) and then all other DB updates can be re-executed. Already existing columns will not be readded, so there shouldn't be any problem with that.

Could it be that you login to a different URL than the one that opens the media selector? If you are logged in to www.example.com, you are not automatically logged on to example.com!
