Which version of the contactform plugin are you using? 1.6 is the recent one.

Whenconfiguring the contactform plugin, did you enter a variablefor the "pagetitle"? It seems you didn't enter any value there?

It seems bug was related with blank field "URL shorthand name" of contact form. Inserting something solved the issue. Is this field mandatory?

Yes, the field is mandatory - I agree, that we are not explicitly stating this.
